  • shotHas your upcoming 7th grade child had his or her shots?  The state of Tennessee requires the TDAP (combined Tetanus, Diptheria, and Pertussis) plus a 2nd Varicella (Chicken Pox) vaccine for entrance into 7th grade.  Students must provide proof of immunization documented on the new "Tennessee Department of Health Certificate of Immunization" form which is completed by a healthcare professional before they can register for 7th grade.  Please make plans now to have your child vaccinated.
